How to submit a nomination

Nominations are accepted from any member of the public, including members of the nominee's family. Nominations are accepted year round, with the deadline for nominations January 31 each year.

You may also include background information of up to 350 words.- Include three letters of support
Letters are required from three individuals other than the nominator. Each letter must not exceed one page. The letters should elaborate or expand on the nominee's achievements or contributions, adding information to assist the Selection Committee in fully considering the nomination.- Be complete
The Selection Committee considers each nomination only on the basis of the written information provided. A nomination will be disqualified if the information requested on the form is incomplete, or if three letters of support are not included. If the nomination is disqualified, the nominator will be contacted by letter. The nomination may be resubmitted the following year unless it is for achievement in competition - these are considered only for the previous year.- Send us your nomination in time

Nominees chosen to receive awards will be contacted prior to the release of their names.
Each nominator will be notified by letter advising whether his or her nominee has been selected for an award. Reasons why a nominee was not chosen will not be provided.
Note: Nominations not selected to receive an award in the year submitted will be held over for up to three years for consideration in each year.
A list of award recipients will be publicized along with an invitation for the public to attend the award presentations.
